Most people working in federal IT have heard the term POA&M before they ever build one. It comes up during ATO pushes, shows up in ISSO job descriptions, and gets referenced in assessment reviews like everyone already knows what it means.
What doesn’t get explained clearly is what a POA&M actually does – not as a concept, but as a compliance artifact that directly influences whether an Authorizing Official trusts your system enough to authorize it.
So what is a POA&M, and what does it actually do inside an ATO?
It’s Not a To-Do List
The Plan of Action and Milestones – POA&M – is a formal document that tracks known security weaknesses against specific plans to fix them. In a DoD or DHA environment, it lives in eMASS. It’s reviewed by your ISSO, your SCA, and your AO. It’s part of the authorization package.
That last part matters. A to-do list has no formal weight. A POA&M is a compliance artifact – it carries scheduled milestones, assigned responsibility, and documented risk. Failing to maintain it accurately doesn’t just leave paperwork incomplete. It affects the risk picture your AO is making decisions against.
NIST SP 800-37 references POA&M maintenance as a continuous monitoring activity. It’s not something you build for the ATO and then archive. It’s a living document that gets updated as findings are remediated, as milestones shift, and as new vulnerabilities surface. The AO’s office expects to see it reflect the system’s current state – not its state six months ago.
What Goes Into a POA&M Entry
Every tracked weakness gets its own entry. In eMASS, that entry has structured fields – and each one carries more weight than it looks like.
The weakness description is where most entries fall short. It needs to explain the problem clearly enough that someone unfamiliar with your system can understand what’s broken, what control is affected, and what the technical condition looks like. “Patch not applied” is not a weakness description. A description that names the missing patch, identifies the affected system type, and maps the gap to a specific control – SI-2 for patch management, for example – is what the SCA is actually looking for.
The mitigation field documents what’s currently in place to reduce the severity of the weakness while remediation is in progress. A weak mitigation statement says “patch will be applied.” A strong one describes the compensating controls actively limiting exposure – network segmentation, monitoring rules, access restrictions – and why they reduce risk in the interim. SCAs read mitigation statements closely. A vague one signals the entry was filled out to satisfy a requirement, not to manage risk.
The scheduled completion date is a commitment, not an estimate. When a date passes without the finding being closed, that’s a flag during review. Missed milestones require a formal extension with updated rationale and revised milestones – not just a date change. A POA&M full of passed dates with no updates reads as a system no one is actively working.
Milestones are the interim steps between identifying a weakness and closing it. If a CAT I finding takes 90 days to remediate, the entry shouldn’t just have a future close date sitting there. It should document what’s happening: patch validated in test environment by week two, change control submitted by week five, production deployment and re-scan scheduled by week eight. Milestones give the AO visibility into whether the plan is real or aspirational.
The assigned owner isn’t a formality. If a finding has no owner, it doesn’t get worked. In most environments the ISSM owns the POA&M and the ISSO maintains it – but whoever is listed against a specific entry is accountable when the SCA asks for a status update.
One category worth calling out separately: IAVM findings – Information Assurance Vulnerability Management alerts – carry DoD-mandated remediation timelines. Missing that window without a POA&M entry in place isn’t just a documentation gap. It’s a compliance failure with its own escalation path.
SysAdmin translation: ACAS comes back with a finding – a Windows Server missing cumulative updates and a STIG deviation for SMB signing disabled. In eMASS, those might become two separate POA&M entries: one mapped to SI-2 for the patch gap, one mapped to CM-6 or CM-7 for the configuration deviation. Each gets its own scheduled date, its own milestones, its own assigned owner, and a reference to the ACAS plugin ID or STIG Vulnerability ID so the finding is traceable back to the scan. Status stays Ongoing until remediation is complete and evidence is attached. Then it moves to Completed – not before.
Ongoing, Completed, and Risk Accepted Mean Something Specific
POA&M entries move through statuses in eMASS, and each status signals something to the people reviewing your package.
Ongoing means the weakness is known and the remediation plan is active. Milestones should be progressing. If an entry has been sitting Ongoing with no milestone updates for several months, it raises questions.
Extensions are what happens when a scheduled completion date can’t be met. This isn’t just a date change – it’s a formal update that requires rationale, revised milestones, and in most environments, approval before the new date is accepted. An extension with a clear explanation and updated milestones reads as active management. A passed date with no action reads as abandonment.
Completed means the weakness has been remediated and you have evidence to prove it. A re-scan showing the finding resolved. A configuration change log. A screenshot of the corrected setting. You don’t close a POA&M entry by asserting that you fixed something – you close it by attaching the artifact that demonstrates it.
Risk Accepted entries are a different category entirely. If a finding can’t be remediated within a reasonable timeframe – or if remediation would break something mission-critical – the AO can formally accept that residual risk. This is documented in eMASS with a rationale and a defined review period. Risk acceptance is not a workaround. It’s a formal decision that the weakness is known, understood, and consciously tolerated.
How the POA&M Connects to the Authorization Decision
The ATO is a risk decision. The AO is looking at the system’s implemented controls, the assessment results, and the open weaknesses – and determining whether the residual risk is low enough to authorize operations.
Your POA&M is the document that answers: what do we know is wrong, what are we doing about it, and who owns the fix? A well-maintained POA&M with current statuses and realistic milestones reads as credible risk management. A POA&M with stale entries, vague descriptions, and passed dates with no updates reads as a system where nobody is actually working the problems.
AOs see both versions. The condition of your POA&M influences not just whether the authorization goes through, but how closely the system gets scrutinized during continuous monitoring afterward.
POA&Ms Don’t Stop After the ATO
Authorization is not a one-time event. Under RMF, authorized systems go through continuous monitoring – which means POA&M maintenance is a recurring responsibility, not a pre-ATO task that ends when the package is submitted.
In practice, this usually means regular review cycles where open entries are checked for milestone progress, status is updated, and new findings from ACAS scans get added as they’re identified. A system that accumulates open POA&Ms without closing them is accumulating documented risk. When re-authorization comes around, a clean and actively maintained POA&M history makes that process significantly more straightforward than one that looks like years of deferred remediation.
The Practical Takeaway
If you’re in a SysAdmin or early ISSO role and working with an RMF package, the POA&M is where the technical work becomes visible to the people making authorization decisions. Here’s what that means practically.
Audit the ongoing entries. Are scheduled completion dates current? Are milestones progressing? Are weakness descriptions specific enough to trace back to actual findings? If the answers are no, those are the first things to fix before any review.
Write mitigation statements that mean something. “Will be remediated” is not a mitigation. Document what’s actively limiting exposure right now – compensating controls, monitoring rules, access restrictions. SCAs read these. Vague statements create doubt about whether the risk is actually being managed.
Map every entry to a control. Every POA&M entry should reference a specific NIST SP 800-53 control. If yours don’t, that’s a gap the SCA will identify. SI-2 for patch management, CM-6 for configuration settings, IA-5 for authenticator management – the mapping should be explicit, not implied.
Don’t mark Completed without evidence. A re-scan result, a configuration log, a change ticket – something has to go in the record that shows the weakness is actually resolved. Marking an entry Completed without attached evidence creates an audit gap.
Treat extensions as formal actions. A missed milestone with documented rationale, revised milestones, and proper approval reads as active management. A passed date with no update reads as a finding in itself.
The POA&M is where your system’s security posture becomes visible to the people who have to defend that posture in front of an AO. Treat it like the authorization artifact it is.
